Question 1:     Do any products which you supply to Airtech International, Inc. contain any Conflict Minerals?

‘Conflict Minerals’ are the following metals and ores:

1.  Tin and Cassiterite.

2.  Tantalum and Columbite-Tantalite for ‘Coltan’T

3.  Tungsten and Wolframite.

4.  Gold.

Question 2:     Do the Conflict Minerals originate from the DRC Region?

The DRC Region’ means the Democratic Republic of Congo and its neighboring countries which are currently Angola, Burundi, Congo, Central African Republic, Rwanda, South Sudan, Tanzania, Uganda and Zambia.

    Yes    Proceed to Question 3

    No     Ignore Questions 3 and 4 - please sign the Declaration

Question 3:     Do the Conflict Minerals originate from scrap or recycled sources?

Answer:        

    Yes    Ignore Question 4 - please sign the Declaration

    No     Proceed to Question 4
